All participants must have an Emergency Notification Form (ENF) on file.

You must complete this form yearly, and this form must be on file in the Continuing Education office no fewer than five days before your child's first class.
If necessary, please complete the Consent for Self-Administration of Medication form (PDF).

Drop-off and pick-up procedures
For the safety of your child, all students 12 and under will need a parent/guardian to come in to drop off and pick up their child to and from the JCCC classroom he/she will be attending. There will be a sign-in, sign-out log you will complete. Call the student enrichment program at 913-469-4420 if you have any questions or concerns. Your child must be picked up promptly after class.
- It is recommended that you arrive 10 minutes before your child’s class begins.
- For course location, please see our campus map.
- Look for staff to direct you to your child’s classroom where you will sign in your child.
- Follow the same procedures for the sign out process.
Golf Students - Drop-off and pick-up is at Smiley's Golf Complex, 10195 Monticello Terrace, Lenexa, KS 66227-4521
(913) 782-1323. Students must bring their own lunch.
Important: If your child is participating in a carpool or is being picked up by someone other than a parent, specific details must be provided in a written note to the JCCC sport clinic staff for every week of the clinic(s).
